Athlete forms & releases

Every form is signed online, time-stamped and stored for Rank Up staff. A parent or legal guardian must sign for any athlete under 18.

Required for any athlete riding in a Rank Up vehicle, RV or tour truck to or from an event.

On-camera interviews, mic'd-up segments, podcasts and any recorded content featuring the athlete.

General event participation, liability release and emergency medical authorization.

Still photography, highlight graphics, flyers and promotional use of the athlete's image.
