I give permission for the athlete named below to ride as a passenger in a vehicle operated by Rank Up, its staff, contractors or media partners, for the purpose of travel to, from or during a Rank Up event, tour stop or media shoot.

I understand travel involves inherent risks. I release Rank Up, its owners, staff, volunteers and partners from liability for injury, loss or damage arising from the ride-along, except for gross negligence or willful misconduct.

The athlete will follow all safety instructions, remain seated and belted while the vehicle is in motion, and behave respectfully toward staff and other passengers. Rank Up may end the ride-along at any time.

I confirm the emergency contact and medical information below is accurate, and I authorize Rank Up staff to seek emergency medical care for the athlete if I cannot be reached.
