I acknowledge that football, combines, showcases and related activities involve risk of serious injury, including concussion, permanent disability and death, and I accept those risks for the athlete named below.

I release and hold harmless Rank Up, event hosts, venues, staff, volunteers and sponsors from all claims arising from participation, except for gross negligence or willful misconduct.

I authorize event staff and medical personnel to provide first aid and to seek emergency medical treatment for the athlete if I cannot be reached, and I accept responsibility for the cost of that care.

I confirm the athlete is physically fit to participate and has no condition that would make participation unsafe, other than any condition noted below.
